Please call our office at (248)723-9200 to schedule a convenient appointment time. Evening appointments are available to accommodate busy schedules. Again, welcome to our family! As you require information about us, we need information about you or your child in order to help. We begin when you first contact us. We are sorry about the many questions, but they are all important. We will either send out a packet or you can download the forms seen below. These can be filled out prior to your visit in the comfort of your own home (our place is nice, but not as nice as yours). The information explains our policies, helps us with insurance reimbursement and helps us provide better care.

Payment Information
It is the patient’s responsibility for payment as well as to contact their insurer to determine the need for a referral and/or pre-authorization. Failure to obtain this information may result in lower payment or denial from the insurance company. For children, the parent authorizing treatment will be the parent responsible for the charges related to that care. In cases of divorce, the authorizing parent is responsible for collecting (reimbursement) by the other parent. Patient co-pays are expected at the time of their appointment. If the co-payment is not known, the patient is expected to pay 20% of the charges for the visit. We will facilitate the processing of all claims; however, account balances are the patient’s responsibility. Accounts are expected to be paid in full within 30 days of a statement being issued. No report will be released until patient balance is paid in full. We participate with many Insurance carriers including Medicare, Blue Cross Blue Shield PPO, Aetna, Blue Care Network, and Health Alliance Plan.
